4 M 2, due to the 6 guy. 9104. 1833 hours, dispatcher 453. Battalion 31 to Brooklyn. Battalion 31. Power box 430. We're going to make this a 43 Code 3. A backhoe that was operating at the construction site. Fell about 25-30 feet below grade. Earlier today.

Someone didn’t do an existing conditions site survey. You can see there was an existing vault/ void in the area the excavator landed in. Probably could not handle the load and improper placement. It also appears to be a very thin slab.
