25-6, available in response, 1950 Cliffside Drive at Juniper Village. Being on the nurse's station on the second floor. 92-year-old female fell out of a wheelchair and struck her head. She has some swelling and an altered mental status. They said that she's also not very responsive. They were able to wake her, but that is not normal for her. 11-52.
